Five Factor Model Rating Form (FFMRF; Mullins-Sweatt, Jamerson, Samuel, Olson, & Widiger, 2006) is a brief, one-page instrument for collecting ratings of the Five Factor Model domains and the 30 facets proposed by Costa and McCrae via the NEO Personality Inventory - Revised. Each of the 30 facets is assessed by a single item that is anchored at both the low and high ends by a set of 2-3 adjectives. The FFMRF was originally designed as method for therapists to describe their clients but has also been used and studied as a self-report measure.
Scoring Instructions: With just a single items, the facet scales are simply scored as the number assigned (e.g., a score of "4"). If you wish to score these for the domains, one needs to simply sum the 6 facets that are included on a given domain.
